Ordering a Test

Before ordering an Individual Genome Sequencing (IGS) service for your patient, we encourage you to contact us to ensure it is appropriate. Informed consent and assurance of pretest counseling by the ordering doctor or a genetic counselor is required.

  • The Sequence Kit Requisition Form will enable you to order a sample collection kit if you already have an interested patient and wish to start the process for them.
The Individual Genome Sequencing service differs from Illumina's research genome service. As described in Illumina's consent form and service agreement, the ordering doctor intends to use the patient's Individual Genome Sequence Information in the management of that patient's care.
